General principles of personal data protection processed by GASTROENTEROLÓG s.r.o. Nové Zámky

Pursuant to Act No. 18/2018 Coll. on the protection of personal data in the wording of the valid regulations and Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, the administrator of this website issues the following policy:

1. We use personal data provided in the forms that are sent via our website exclusively for the purpose of making appointments regarding medical examinations or providing advice within our virtual office.
2. All personal data sent electronically via our website,  are irretrievably deleted from our information technology (IT) system after being used for the above purposes.
3. Each person applying  for advice or electronic appointment for themedical examination must give consent to this policy by ticking the appropriate box on the form before submitting it.
4. Each patient who visitour outpatient office in person must sign a consent form for processing personal data, otherwise the healthcare cannot be provided.
5. This consent can be withdrawn at any time by the applicant electronically, by sending an e-mail to geanz(at)gastroenterolog.com or by SMS to 0940 890 823 containing text "I withdraw the consent to the processing of personal data that I have given by submitting the form via the website. My data in the form were as followed: Name or nick..... e-mail........." In the message  it must be clearly stated who is requesting the withdrawal of consent. This can only be done by the person who has given the consent. This withdrawal of consent applies exclusively to the data in the virtual office, since the deletion of personal data from the archive in our outpatient office is possible only after 20 years since the last examination. .

Who processes your personal data? The processing of personal data is carried out by the owner and managing director of the company- MUDr. Jozef Klucho.

Why do we process your personal data? We need to process and collect your personal data in order to carry out our activities, to provide professional advice in virtual and medical services online and in our outpatient office. Without your personal data, this activity, which we also carry out at your request, would not be possible.

What data do we process? In the virtual office and in the order form, your name will be replaced by your initials whileyour age will be stated, in order to make it more difficult to identify the sender. Only the first name without the surname, or nickname (nickname you choose yourself)and the text of your question, consisting of a description of your difficulties, the medical examinations carried out, the previous treatment and overcome diseases will be publicly accessible, without disclosing the sender's email or IP address. This data are accessible only to the owner of the website and the web hosting provider, or to persons authorised by them, who are bound by the web hosting provider's internal data protection guidelines. In our outpatient office we process the first name, surname, personal identification number, permanent residence, telephone contact, the name of the person to whom the patient's information can be disclosed, the telephone contact of that person and the patient's health insurance company. In addition, we process all data relating to your medical examination.

Who have access to your personal data? MUDr. Jozef Klucho and the current web hosting provider Webglobe, a.s., having its registered office at Stará Prievozská 1349/2 821 09 Bratislava - Ružinov, www.wy.sk have access to the personal data sent via the forms on our website www.gastroenterolog.com. . The owner of the web hosting server and persons authorized by him are responsible for the handling of personal data on the web hosting server., In the outpatient office, the nurse  (in present Ágnes Urbanová) also has access to the personal data.

To whom do we provide your personal data? In the virtual office, we process all personal data exclusively with your consent and disclosure to third parties is excluded.
In the  outpatient office, we process your data on the basis of your request for the medicalexamination and provide it to the following entities, if necessary
- your health insurance company
- medical professionals and medical institutions whose assistance is necessary for your medical examination and treatment.
The disclosure of your personal data for commercial purposes is excluded.

What do we do with your personal data? In the virtual office, the data of people applying for advice in our consulting room are published anonymously. Attachments containing sensitive data concerning health are deleted from our information system and from the webhosting server immediately after the response has been processed. In our outpatient office we process data in paper and electronic form and archive them for 20 years in accordance with the law.

What rights do you have in relation to the processing of your personal data? You can request information on how we process your personal data, including access to them, unless they have already been irretrievably deleted (e.g. from the virtual office). If you are concerned that we are processing your personal data in an unlawful manner, you have the right to

-ask us for an explanation or make objection to the processing of your personal data

request redress or erasure of your personal data

In this case, we can only comply with your request within the virtual office,. since we are obliged by law to archive personal data and medical documentation in the outpatient office for 20 years even if you are no longer visiting our office. 

-file a complaint with The Office for Personal Data Protection of the Slovak Republic

We will inform you of your objection, which must be made in writing, within 30 days of receiving it by post or email.
